- License / Licencia
- Physical Records from Physician (not older than a year) / Exámen fÃsico del Doctor (no mas viejo de un año)
- Immunization Records from Physician (up to date-original) / Certificado de Vacunas del Doctor (que este al dia – original)
- Florida Power & Light Bill / Cuenta de la Compania de Electricidad
- Lease Agreement, Mortgage Deed, or Property Taxes (one of these three) / Contrato de Alquiler, Contrato de Compra, o Taxes de la Propiedad (uno de los tres)
